Acknowledgement & Assumption of Risk

    1. RISKS INHERENT TO THE ACTIVITY/STAY

    I have been informed of the risks inherent to the activity/stay in which I will participate, and these are, in particular, but not limited to;

    • Injuries due to falls or other movements, (sprain, strain, fracture, etc.);
    • Injuries with blunt or sharp object (branches, material, etc.);
    • Cold or hypothermia;
    • Contact with water or drowning, debris or rocks in water, variations in depth; site is not lifeguarded
    • Burns and/or other heat induced injuries including smoke and gas
    • WINTER: Ice, snow, mud
    • Do not walk on the lake. If you do, it is at your own risk

    2. CONFIRMATION OF INFORMATION AND ASSUMPTION OF RISKS

    I hereby certify that the information submitted in this waiver is, to the best of my knowledge, exact and accurate. I am aware that the activities offered by Les Refuges Perchés take place in semi-wild or natural environments that, consequently, are quite distant from medical services. This could result in long delays during an emergency requiring an evacuation and, as such, a possible aggravation of me or my parties state of health or my injury. I acknowledge that I am informed about the risks inherent to the activities and I (and all members of my party) am (are) able to participate in the activity or the stay WILLINGLY AND I ACCEPT ANY AND ALL RISKS THAT such an activity or stay can comprise. I also pledge to play an active role in risk management by adopting a preventive behavior with regards to my (my parties) own safety, and the safety of the other persons that surround me.

    3. AUTHORIZATION IN CASE OF EMERGENCY*

    I, undersigned, authorize Les Refuges Perches, if necessary, to take decisions in case of an accident to transport me (or members of my party) (by ambulance, ATV, toboggan or boat or other) to a hospital or health care center.

    4. DAMAGE & CLEANING SECURITY DEPOSIT: Please return the treehouse as you received it.*

    A $190 security deposit will be required to pay after signing this for. It will appear on the 2nd page. This pre-authorization “holds” the amount and does not actually charge it. At your departure, a verification of your treehouse will be made. The amount will be released and will disappear from your “pending transactions” on your statement between 5 and 7 days, depending on your bank's processing time. In winter, stairs and decks of treehouses are to be cleared of snow, otherwise they become compacted after walking on it and staff can no longer remove and manage the build-up for the next guest. Charges can be taken from the security deposit if the treehouse and treehouse property was not cleaned and found as you received. Charges can also be taken if there are any damages or missing items relative to the treehouses, park or property. Please, no smoking in treehouses. Thank you in advance for the respect you will show to our environment, treehouses, park and to your fellow guests.

    (if you have a dog) 5. DOG OWNERS CODE OF CONDUCT / Please read our guidelines below

    I have been informed of the risks inherent to the activity/stay in which I will participate, and these are, in particular, but not limited to;

    • We have a mission to ensure the protection and conservation of the Park environment.
    • When dogs are loose and off trail, they quickly destroy the natural forest coverage and push wild animals away from their natural habitat. We have also had many dogs encounter porcupines when off leash.
    • Many people are scared of dogs even if they are cute.
    • Some dogs do not tolerate other dogs, so if your dog is off leash, it can cause a fight when approaching another dog on a leash, or vice versa.
    • Due to Park regulations, all dogs must be on a leash at all times.
    • By keeping your dog on a leash you will make it easier for you to clean-up after it.
    • Always pick up your dog’s droppings.
    • Ask us for biodegradable waste bags that you can place in black garbage bins in main parking lot.
    • Please keep dogs off the beds to avoid damaging the vinyl bed covers and to help other guests with dog allergens.
    • Before check-out please do the tour of your treehouse to see if any droppings have been left behind – respecting the next visitor.
    • Did you forget a leash or a rope to give your dog more liberty around your treehouse? We will be happy to lend you one.
